What is consent?

• Consent is your verbal or written agreement to allow a counselor, outreach worker, clinician, or medical professional to provide you with various health services, etc.
• You could go through the consent statement in details or ask the counselor or health professional to explain you more about the procedure and information that is needed from you as part of agreeing to join the procedure.
• If you don’t feel comfortable about sharing certain information, let the counselor or health professional know, and you can always opt out of talk.

Can I trust you with my result or identity?

• One way for us to protect your privacy is to create a unique ID code – or UIC – for you. No one else has the same code as you do.
• There are instances where we might need contact information – such as phone number(s) – from you for follow-up purposes. Such information will not be used for commercial or any other purposes harmful to you or your privacy.Contact us if you have any questions regarding privacy and confidentiality.
